To apply for Membership with Millennium Corporate, you may complete the online form or select the Membership Application PDF and complete the off-line form. Upon application approval, the credit union must complete and submit the Resolution Authorizing an Institution to Open/Maintain Accounts and Use Services. Find these forms in the Membership Forms section below. All completed forms need to be submitted to our Member Engagement Team.
